Our questions help route your request. A message such as "You qualify" means only that your answers meet the basic criteria for review by a participating firm, not that you have a valid claim.

Your Responsibilities
You agree to provide true, accurate information about yourself, to submit only your own information, and to use a phone number you own or are authorized to use.

If you check the consent box and submit our form, you agree to receive marketing calls and text messages from Work Injury Support and its marketing partners, including by automated technology, artificial or prerecorded voice and AI-generated voice, even if your number is on a federal or state Do-Not-Call registry. Consent is not a condition of purchase. Reply STOP to opt out.
Electronic Signatures
You agree that checking a box and clicking Submit is your electronic signature and has the same effect as a handwritten signature.
MANDATORY ARBITRATION AND CLASS ACTION WAIVER

Any dispute arising from or relating to this site, our form or any communications from us or our marketing partners will be resolved by final and binding individual arbitration administered by a national arbitration provider under its consumer arbitration rules, except that either party may bring an individual claim in small-claims court.
You and we waive the right to a jury trial. Claims may be brought only on an individual basis, and never as a plaintiff or class member in any class, collective or representative action.
You may opt out of this arbitration agreement within 30 days of your first form submission by emailing support@workinjurysupport.org with your name, phone number and a statement that you opt out of arbitration.
If the class action waiver is found unenforceable for a claim, this arbitration agreement is void as to that claim. The Federal Arbitration Act governs this section.
